- On discharge, the subscriber has to sign on the claim form presented and filled by the hospital \ Medical Centres, where all the services done for him\ her is mentioned.
- Whenever inpatient admission is advised, weather for surgical intervention or medical follow up, Modern Int’l Healthcare Services Hot Live Service supports all required preauthorization for such cases around the Clock.
Medical Consultation:
- XYZ Card - Holder can visit any of the medical centre enrolled. We will fix the appointment for the patient as per case to case basis.
- Within the network of service providers at any of the Medical Centres, Poly Clinics mentioned or Hospitals.
- The subscriber complete the subjective part of the claim form, where the staff at the medical establishment complete the rest.
- XYZ CARD Holder has to settle any deductible amount mentioned on his\ her Card prior to Medical Consultation.
- Any further procedure required for the subscribers’ medical case are carried out directly.

- The Subscriber has to sign on the Claim Form for all the services presented at the medical establishment.
Medical Investigations:

- The Pink Copy of The claim Form is specific for Radiology Procedures (As X Rays, Ultra Sonography, CT Scan or MRI, etc.)
- The Blue Copy of The claim Form is specific for Laboratory Procedure (Blood Tests, Urine Tests, Specimen analysis, etc.)
- XYZ CARD Holder can directly claim the prescribed service from the nearest Diagnostic Centres (XYZ CARD Holders can always inquire about the nearest Centres using Modern Int’l Healthcare Services (Hot Line Service)
- Whenever a preauthorization is required for any of the prescribed procedures, Modern Int’l Healthcare Services Hot Line Service supports such requirements around the Clock.
Dispensing Medication:
- Through any of the enrolled Pharmacies, which are distributed all over The United Arab Emirates, XYZ CARD Holder can dispense prescribed medications using The Yellow Copy, Form filled by the treating Doctor.
- XYZ CARD Holders can always inquire about the nearest pharmacy \ 24 hours service pharmacies using Modern Int’l Healthcare Services Hot Line Service.
- XYZ CARD Holder has to settle any Co-Insurance percentage mentioned in the Pharmaceuticals box on his\ her card on dispensing the prescribed medication.
